You have a right to receive a "Good Faith Estimate" explaining how much your medical care will cost. Under the law, healthcare providers are required to provide patients without insurance, or those not using insurance, with an estimate of costs for medical items and services.
You can request a Good Faith Estimate from me, or any other provider you choose, prior to scheduling a service.
If you receive a bill that is $400 or more above your Good Faith Estimate, you have the right to dispute the charge.
Be sure to keep a copy or photo of your Good Faith Estimate. For more information about your rights regarding Good Faith Estimates, visit www.cms.gov/nosurprises.

If you are using insurance, you will also receive a separate link to complete additional forms in my billing portal. Please ensure that you complete all paperwork at least 48 hours before your session to allow sufficient time for processing.
